Description. This prospective population health research project is exploring how a virtual fully integrated health and social care program could be organized and governed to optimize access, cost and impact at a national level. We envision a future where every person and family in Canada can access the health, emotional, social and spiritual support that is right for them in order to live well to their natural end of life within communities that are compassionate, connected and equitable.
Key Activities:
We offer the following services via video or over the phone to people across Canada:
- We engage people in quality-of-life conversations, wellness checks and on-going research.
- We administer social health risk screening tools, assist people in completing their own quality of life self-assessments, and help them navigate community resources to build flexible “Community Circles of Care”
- We will track population level quality of life outcomes and change over time –including measuring the value of preventative care
- We offer virtual public education related to integrated health and social care
- We research how student experiential learning in this field impacts on knowledge, attitudes and behaviour of future health care professionals.
