What We Do
  • Design, test and pilot new tools, application ready solutions and algorithms that meet communities’ rapidly changing technology needs
  • Provide access to a common platform/ e-technologies to build shared HWFC dataset, and manage dataset on behalf of affiliates
  • Track quality of life patterns and impact to contribute to longitudinal population health research

University of Windsor – HWFC Community Information Management System

Purpose

We are working to develop a set of innovative person-directed technologies that organize data from multiple services and care providers around the person within their family and community, putting them at the centre of an integrated, personalized care network so that health, social services, and informal supports can be optimized to sustain quality of life. By mapping assets, identifying social risks and making patterns of care and health visible across neighbourhoods and care settings, citizens and organizations learn from each other and are motivated to take action to improve, utilizing their unique strengths.
